Membership suspend reqeust form

Elite Taekwondo Center (Palo Alto Martial Arts)s’ policy is designed to best suit students’ needs. Our make-up policy provides students with opportunities to make up missed classes. This prevents students from falling behind and allows students to stay on schedule with belt promotion.

If a student has to miss classes for more than a month, the membership will be extended for only a month. A student must notify instructors at least two weeks before the scheduled absences to be able to extend the membership. The membership will end after the extension. Please note that an extension will only be given if at least 4 consecutive weeks of classes are missed.

If a student has to miss classes due to medical reasons, the student must receive a written explanation from a doctor. The membership will then be extended according to the doctor’s recommendation.
